In combination with pathological examination, CT and MRI have been used to evaluate the efficacy of PSC, they generally coincide well with pathological evaluation. We here present two cases showing discrepancies between pathological examination and imaging analysis after PSC in breast cancer. We should keep such discrepancies in mind to determine the ideal treatment after PSC. An accurate method of evaluating cellular damage by PSC is needed. Duct lavage cytology was originally established to reveal groups at high risk for breast cancers by detecting metaplastic ductal cells. We report here a case where duct lavage was useful for revealing a small cancer that had been undetected by repeated bloody nipple discharge and cytological examinations. Duct lavage cytology may be of use in cases where nipple discharge of unknown origin persists. It was easy to introduce the guiding needle into breast cancers smaller than 1cm under ultrasonographic observation. Neither bleeding nor infection was experienced. Wound management was easy and painless. This guiding system is very useful for locating and managing nonpalpable breast cancers even in outpatient settings. The first case was a 58-year-old male. He underwent a subtotal esophagectomy and reconstruction using the stomach via the subcutaneus route. His postoperative course was stable; however, 13 days after the operation、 he had an elevated fever and pulmonary thrombi were observed on CT-scan screening. Scintigraphy confirmed the thrombi. An inferior vena cava filter (IVCF) was inserted and anti-coagulation therapy using heparin was started. His fever subsided, and 35 days after the operation, the thrombi disappeared. He was discharged thereafter and is being followed up using warfarin. The second case was a 74-year-old male. He also underwent subtotal esophagectomy and reconstruction using the stomach via the subcutaneus route. A central line was inserted via right femoral vein; however, the artery was injured and pressurization to the vessel was needed. His post-operative course was stable and symptom-free; however, ultrasound screening detected DVT in his right femoral vein. IVCF was mserted and anti-coagulation therapy was begun, thrombi disappeared 8 days after the therapy. Among them, 12 cases and 12 lesions were squamous cell carcinomas. Although perforation complicated this procedure in one patient and hemorrhage in another, these were improvde by conservative treatment. Marked subcutaneous emphysema occurred in the neck and face during EEMR without esophageal perforation in one patient. Therfore the procedure was stopped and the patient underwent radical operation 41 days later. Two of the 12 patients with carcinoma underwent radical operation 30 days after EEMR because pathological study revealed m3 and ly1 in one them and sm1, ly2 and v1 in the other. Radiotherapy was indicated for another patient who had m3, ly0 and v0. In the other patients, the cancerous lesions devaloped within m2, ly0 and v0 except in one who had minimal m3 invasion, and the therapy was completed with only EEMR. The mean period of observation after EEMR was 15.1 months (7-24). Follow-up did not reveal any recurrence of carcinoma in these 9 patients. LoVo cells were very sensitive to IFN-γ and slightly sensitive to IFN-α and resistant to TNF-α in vitro by dye uptake method. Intravenous administration of IFN-γ significantly inhibited tumor growth transplanted subcutaneously into nude mice, but IFN-α and/or TNF-α did not show any antitumor effect. Labeling index on stanining with bromodeoxyuridune and mitotic index of LoVo cells treated with IFN-α,γ or TNF-α differed slightly from those of the control group. We previously reported that the synergistic antitumor effects on the three cell lines could be examined by the combined use of IFN-α and TNF-α and the mechanism of the synergism was arrested in the S phase of cell cycle of target cells. However in the cese of LoVo cells, the combined use of IFN-α and TNF-α was not effective and did not arrest the cells in tha S phase of cell cycle. Therefore, arrest in the S phase was suspected to be responsible from the synergistic antitumor activity of IFN-α and TNF-α on the sensitive targets. The patient was a 59-year-old male with the chief complaint of left thoracic pain. The 57×43×35 mm tumor was in a region centering on segment 6 of the left lung and had infiltrated the descending aorta. It was diagnosed as T4NOMO, Stage IIIb. A bypass for blood flow was established between the aortic arch avobe the invasion and the left femoral artery, after ligation of the pulmonary artery and vein and closure of the left bronchial stump. Vascular blocking forceps were applied to the descending aorta above and below the tumor infiltration, and two thirds of the circumference of the invaded aortic wall, 4.0×3.0 cm, along with the tumor, was resected. Reconstruction was performed with an 18mm Cooly double velour graft. Blood pressure was monitored with a pressure probe inserted into the right femoral artery. It did not fall below 60 mmHg during the operation. Local recurrence was observed 10 montns post-operatively, and the patient is now receiving radiation therapy. The patient was a 73-year-old man. We administered chemotherapy of CDDP+VP-16, and performed an operation after 2 courses of this chemotherapy. Subtotal esophagectomy and reconstruction with the small intestine was performed. More than three years after resection, he remains alive and recurrence-free. There are few cases of esophageal carcinosarcoma and small cell carcinoma. We report this rare case herein.
